Neat. Can this also be done on a player faction level? That is, could a player-run faction be friendly or allied with an NPC faction, or is it just on a person-by-person basis?

So long as any such blacklist is configurable by server admins, I agree. I say that because of a lot of experience with overzealous filters in other games.

Several of you seem to have missed the bit where I suggested configuration options that could limit renaming on a server to only admins. I'm well aware of how some people could abuse this feature.
