You might want to state the (more or less obvious) purpose of the faction, and some rules, especially regarding multiple memberships[one member being in other factions too].

My only question regarding the application is: Why do you need the age? People can just lie about it and it also states next to nothing about how mature someone behaves. The only reason to know it IMO would be legal reasons, but then I wonder why a faction needs to worry about that.
