Depends on how your server works. All I know is that with our server the person we rent the box from has to drop the update into a folder so we can force the update server side with our server toolset. Otherwise, no clue.

Very much agree with this. Though personally I have little interest in a plex door. Hitting a closed gate is less interesting than someone jumping through a gate only to find the other side littered with turrets and drones :)

I think a lot of this stuff is really up to the server Administrators than "this needs to be part of the game" type of attitude. A lot of these things, including random locations, derelict ships, systems with more materials, we do on Cosmic Pantheon in order to keep the smaller factions growing...
