Bug Planet segments with holes in them

Has anyone else had this happen? Giant holes all the way to the core in the center of each planet segments, and often large raised portions of planets just beside the holes.

Please let me know if you have had the problem as well, that way I can file a bug on the tracker.

I have seen this on my server, and one of my admins confirmed it isn't a visual glitch as he could also see it. This is a bug

I have not seen the holes, but I definitely have seen sudden mountains that have literally no smoothness, a lot like the ones in the second picture. I'm 30% sure its (The mountains) intended, but it looks AWFUL
