Today, it happened to me twice that I started at the world spawn after starting the game, with default credits and default inventory.
I am playing SP with cheats and my creations are still there, so nothing really got lost. But having to recheat all block types that a I need everytime I decide to build for a few minutes is annoying. Furthermore, people playing "honest" might encounter this too, so this is an issue.
I am exiting the game via the EXIT button in the game menu. I am starting the game via the launcher, doing nothing but clicking OK in the settings screen (playing SP sandbox, as mentioned above).
The universe is a new 0.14 universe (I'm on 0.1401).
