While you are in flight mode, sometimes, a bug happens that you can not turn, and you're piloting in freecam mode, and rotation does not show up, though it still rotates. This has happened to me once.

Are you using right shift when in flight? double pressing right shift toggles free cam, holding it is temporary.
Possibly you are talking about the F11 (press F11) mode too? That one randomly triggers for me every few weeks, still don't know what causes it :d
